Domestic Abuse Related Death Review (DARDR); formerly Domestic Homicide Review (DHR)

 A decision to undertake a DARDR, formerly known as a Domestic Homicide Review (DHR), under section 9(1) of the Domestic Violence, Crime and Victims Act 2004, is taken when the death of a person, aged 16 years, or over has, or appears to have, resulted from domestic abuse. 


A DARDR seeks to review the circumstances in which the death occurred, with a view to identifying the lessons to be learnt from the death[1].


It is the responsibility of the Community Safety Partnership, located within each Local Authority, to appoint an Independent Review Chair and Author. 

DARDR Independent Review Chair and Author Services

Rebecca is a Lead Safeguarding Reviewer (accredited by the University of Portsmouth and the Criminal Justice Institute) and undertakes Domestic Abuse Related Death Reviews (DARDRs) within her independent practice. 


Rebecca has completed the Home Office Domestic Homicide Review (DHR) online training, including modules on chairing reviews and producing overview reports. She has completed further DHR specific training with Advocacy After Fatal Domestic Abuse (AAFDA) ‘Managing a DHR’ (Domestic Homicide Review) and is due to attend the Home Office DHR Chair Training with AAFDA in early 2026. 


Rebecca has extensive experience working with families impacted by domestic abuse and has a specialist practice interest in the prevention of harm connected to domestic abuse. She is a member of the AAFDA Domestic Homicide Review (DHR) Network.
